As part of Self Care Week the NHS is encouraging all Brits, but especially those with a long-term condition, to take control of their own health and wellbeing.
The NHS said that living a healthy lifestyle is an important part of self care. It acknowledged that this can be a challenge for those with a long-term condition but said that making the effort to put on gym clothes and exercise regularly will make a big difference to their wellbeing.
GPs and other healthcare workers can suggest changes that are suitable for people with different conditions and any treatment that patients are having.
Health or care workers can put patients on the Exercise on Referral scheme, which is run with local gyms and fitness centres and can include low impact exercises such as swimming.
The NHS website provides information on how to lead a healthier lifestyle and also has interactive tools to help people find out whether their diet is healthy, if they are the right weight and if they need to put their gym wear on and exercise more often.
